Alpha-lipoic acid has the deeper trial programme (pooled IV trials, oral RCTs, and the four-year NATHAN 1 study), with a ~24% relative symptom improvement over placebo but no effect on nerve conduction or NATHAN 1's primary endpoint. Acetyl-L-carnitine has two positive pooled RCTs in the same population, though a 2019 Cochrane review still rated the certainty 'very low.' Different mechanisms — antioxidant vs. mitochondrial fatty-acid shuttle — the same wall.
