Before you pay: what the evidence says
The first-line, best-evidenced treatment for erectile dysfunction is a PDE5 inhibitor — sildenafil (generic Viagra) or tadalafil (generic Cialis). They work on blood flow, have decades of trial data, and as generics they are cheap. Most online ED services are, at their core, a convenient way to get a prescription for one of these. A second class of treatments works on the brain instead: the dopamine agonist apomorphine and the melanocortin peptide PT-141 (bremelanotide) act centrally to initiate arousal. These are real mechanisms but less effective than PDE5 inhibitors, so they show up mostly as adjuncts or in combination products. Finally, botanicals like horny goat weed (icariin) are marketed as “natural Viagra” but have only weak, mostly preclinical support. Be cautious with compounded combination products that add ingredients beyond approved PDE5 inhibitors: they can be reasonable, but the added components are less proven.
