Tirzepatide is approved and pemvidutide is Phase 2, which decides the practical question outright. On magnitude tirzepatide also leads — about 21% in SURMOUNT-1 against roughly −15.6% in the 48-week MOMENTUM trial — though those are separate trials in different populations and not a head-to-head. Pemvidutide’s argument is deliberately not the headline number: Altimmune reports about 74.5% of the weight lost was fat and 25.5% lean, and frames the glucagon arm as protecting body composition. That is best read as the hypothesis the program is built around rather than a demonstrated advantage, for two reasons — it is a company-reported ratio from a Phase 2, and lean mass on a scan is not the same thing as function preserved. Pemvidutide does carry Phase 2b MASH data, an indication tirzepatide is not approved for.
