Its terms list all fifty states plus DC, Guam and Puerto Rico, subject to a provider being licensed and available where you are. The roughly $75 monthly starting point in its FAQ describes hormone optimization, not a priced peptide or TRT prescription. Ask for the Annual Care Plan, medication, lab and platform fees together before committing.

Evolve Telemed is a hormone- and peptide-optimization telehealth practice serving men and women, with a Bloomfield Hills, Michigan business address and tracks covering TRT, weight management, sexual wellness, hair restoration, energy, skin, and — for women — menopause and BHRT and thyroid. It describes itself as a management platform supporting independent board-certified providers, and names physicians on its site, including Dr. Leita Harris, MD, a board-certified gynecologist. Its treatment catalog lists 33 products, among them tesamorelin, GHK-Cu, glutathione, NAD⁺ injections, SS-31, PT-141 nasal spray, 5-Amino-1MQ, enclomiphene, testosterone injections and cream, hCG, nandrolone, oxandrolone, compounded semaglutide and tirzepatide, metformin, tesofensine, rapamycin, modafinil and methylene blue. It shows a LegitScript seal and publishes press, blog and FAQ resources.

A free consultation leads to LabCorp testing and a provider video visit. Patients in New York, New Jersey, Connecticut and Rhode Island are directed to order and pay for their labs through dedicated LabCorp links, then upload results; they are not simply excluded from testing. Evolve says Annual Care covers the initial consultation, an annual visit, provider-requested follow-ups and coordinator access. Its terms make that plan nonrefundable after the first consultation, while compounded medication is nonreturnable. The same terms separate provider charges from platform administration charges. State eligibility depends on licensure, patient location and provider availability despite the broad jurisdiction list. Pharmacy partners remain unnamed.

The FAQ says an effective hormone optimization plan starts around $75 a month for many patients. That is not a price for a named peptide, TRT product or Annual Care Plan. A $25 first-treatment promotion appears on product pages, but the underlying product cost is not published. Request the initial debit, Annual Care price, medication, platform fees, lab charges and renewal terms in one quote. New York, New Jersey, Connecticut and Rhode Island patients have a separate self-pay LabCorp route. Prescription quantity and shipping time depend on the selected order.

Compounded; not FDA-approved. Prescription required only if a licensed provider finds it appropriate. Evolve does not name its partner compounding pharmacies, and compounded sterile products are not reviewed by FDA for purity and potency the way approved drugs are. The named catalog sits on very different evidence. Tesamorelin has genuine approved human evidence, but for HIV-associated lipodystrophy, not general wellness, and a compounded version is not the approved product. GHK-Cu, glutathione and SS-31 have essentially no controlled human outcome trials for the uses they are sold on. Injected NAD⁺ has none either; the studied forms are the oral precursors NR and NMN, which raise blood NAD⁺ rather than demonstrating a clinical benefit. PT-141 is the exception, with registrational trials for hypoactive sexual desire disorder in premenopausal women. Testosterone therapy has a genuine evidence base for men with diagnosed hypogonadism and carries real obligations: hematocrit, PSA and lipid monitoring, and suppression of fertility that can persist. Enclomiphene is a different drug that raises your own testosterone and generally preserves fertility. Nandrolone and oxandrolone are anabolic steroids rather than replacement therapy, and their presence on a hormone-optimization menu is worth asking about directly. On the weight side, compounded semaglutide and tirzepatide are both listed. Individual results vary.
