Our verdict
Both sit in the same clinical tier, and neither has been compared with the other directly. Ashwagandha has the deeper stack: multiple placebo-controlled RCTs of standardized extracts, KSM-66 and Shoden, showing lower perceived stress and cortisol, decent sleep data, and a modest, thinner testosterone signal; it also carries a rare but documented liver-injury case series. Tongkat ali's human trials are fewer and smaller, with a randomized trial in moderately stressed adults reporting lower cortisol alongside higher testosterone, and the clearest hormone effects in men who were already low; its practical weakness is that eurycomanone content varies enormously between products. Trials on both sides are small, short, and often industry-linked, and neither is a substitute for treating diagnosed hypogonadism.
