CagriSema vs Semaglutide

CagriSema is semaglutide with an amylin analog added in the same weekly shot; the question is whether the second pathway earns its place, and it is not approved.

Our verdict

Semaglutide is approved and has the deeper record: about 14.9% mean weight loss in STEP 1, the STEP 4 data showing substantial regain after stopping, and a roughly 20% reduction in major adverse cardiovascular events in SELECT. CagriSema reported about 20% in REDEFINE 1 and also beat placebo in REDEFINE 2, but it is investigational, has no long-term cardiovascular outcomes evidence, and its topline landed below pre-trial expectations, partly because many participants did not reach the highest target dose. REDEFINE 1 did randomize against the individual components as well as placebo, which makes it the closest thing to a direct read in this pair.

CagriSema fits if

You are watching whether pairing amylin with GLP-1 becomes an approved option, and you read Phase 3 data before it reaches the clinic.

Semaglutide fits if

You want a GLP-1 that can actually be prescribed, with hard cardiovascular outcome evidence behind it.
