Both can be prescribed — orforglipron was FDA-approved for weight management in 2026 as Foundayo — but semaglutide has by far the deeper evidence base, with STEP 1 plus the SELECT cardiovascular result. On weight, semaglutide reported about 14.9% in STEP 1 against roughly 11% for orforglipron in ATTAIN-1, though those come from separate trials and were not compared directly. Orforglipron's real argument is form: a non-peptide small molecule taken as a once-daily tablet with no food or water restrictions. It has no long-term cardiovascular or renal outcomes data yet, and its type-2 diabetes indication is still under review.

Orforglipron fits if

You want a GLP-1 you can swallow rather than inject, with no food or water timing rules, and you accept a shallower outcomes record for that convenience.

Semaglutide fits if

You want a GLP-1 available now, with pivotal weight-loss trials and a dedicated cardiovascular outcomes trial behind it.
