These are not alternatives so much as two heights on one ladder, and the lower rung is the better-understood one. DHEA has genuine replacement value in true adrenal insufficiency and is FDA-approved as intravaginal prasterone, but its anti-aging trials were negative and it barely moves testosterone in healthy young men. Pregnenolone sits above it, upstream of the entire pathway, which sounds like more leverage and is actually less control: an oral dose branches toward progesterone, toward DHEA and the androgens, or toward corticosteroids according to tissue enzymes rather than intent. Its randomized trials are adjunctive psychiatry, and the one study best linking a dose to a brain effect measured allopregnanolone — a downstream metabolite the label never mentions. Both alter the steroid markers used in doping control.
