Our verdict
These are not two versions of the same thing, and they have not been compared head to head. TRT is FDA-approved for diagnosed hypogonadism, meaning symptoms plus unequivocally and consistently low testosterone confirmed on repeat morning testing; in the Testosterone Trials it improved sexual function moderately and mood slightly, with no significant benefit for vitality, and in TRAVERSE major adverse cardiac events occurred in 7.0% of the testosterone group versus 7.3% on placebo. It also suppresses your own production and sperm output. DHEA genuinely helps people with adrenal insufficiency and is FDA-approved in one specific form, intravaginal prasterone for painful sex after menopause, but the 2-year Mayo Clinic trial found no benefit for body composition, physical performance, or quality of life, and it does not meaningfully raise a healthy young man's testosterone. DHEA is not a softer, over-the-counter version of TRT; it is also banned by WADA.
DHEA fits if
Someone with a documented DHEA deficiency, or a postmenopausal woman whose issue is painful sex and who is discussing the approved intravaginal form with a clinician.
Testosterone (TRT) fits if
A man with symptoms and unequivocally low testosterone confirmed on repeat morning labs, who understands the fertility trade-off and wants monitored therapy rather than a supplement.
