A real, selective free-radical mechanism from 2007 — but the flagship Parkinson's story is a three-act replication failure (positive pilot, null 178-patient multicenter trial, null inhalation trial by the same investigators), and a 2026 meta-analysis of 13 RCTs called the pooled metabolic-lipid effects 'clinically modest,' with several of the more favorable trials carrying industry or device-maker ties.

Our verdict

Dietary nitrate has the deeper, more consistent record — a genuine ~4-5 mmHg systolic blood-pressure effect and a real endurance benefit — but one well-documented to fade in elite athletes, and one antibacterial mouthwash can abolish outright. Molecular hydrogen's mechanism (selective free-radical scavenging) is more novel, but its flagship Parkinson's story failed to replicate twice, and its endurance trials show the same fitness-dependent pattern nitrate does, on a thinner overall evidence base.

Dietary nitrate (beetroot) fits if

You want the more established, better-replicated option, and you're a recreational rather than elite athlete, or you're managing blood pressure.

Molecular hydrogen fits if

You're specifically interested in the selective-antioxidant mechanism rather than the nitric-oxide pathway, understanding the human trial base is thinner and more mixed.
