Our verdict

In DURATION-6, once-daily liraglutide 1.8 mg beat once-weekly exenatide on both HbA1c (a reduction of about 1.48% versus 1.28%) and weight (roughly 3.6 kg versus 2.7 kg). The outcomes data point the same way: liraglutide cut major adverse cardiovascular events by about 13% in LEADER, while exenatide was safe in EXSCEL but showed no statistically significant reduction. Both are approved for type 2 diabetes; only liraglutide is also approved for chronic weight management. Exenatide's significance today is mostly historical.

You have a specific reason to stay on exenatide, such as availability or an established individual response, and glucose control is the goal.

Liraglutide fits if

You want the stronger of the two on glucose, weight, and cardiovascular outcomes, and can accept a daily injection.
