Liraglutide is the molecule that proved a GLP-1 receptor agonist could do more than lower blood sugar — it could reliably take weight off. It reached the market first, as a once-daily injection: Victoza (1.8 mg) for type 2 diabetes in 2010, then Saxenda (3.0 mg) for chronic weight management in 2014.[5][6] A decade later it is the honest baseline against which the weekly drugs are measured. The results below are real and durable — and, on the scale, now clearly eclipsed by semaglutide and tirzepatide.
What it is, and how it is dosed
Liraglutide is a first-generation GLP-1 receptor agonist with a plasma half-life of roughly 13 hours — long enough for once-daily dosing, but not the once-weekly cadence of the molecules that followed. That difference is the whole story of its clinical position. It is marketed under two brands at two doses: Victoza, titrated to 1.8 mg daily for type 2 diabetes,[6] and Saxenda, titrated over weeks to 3.0 mg daily for chronic weight management.[5]Both are subcutaneous injections with a stepwise dose escalation designed to blunt the gastrointestinal side effects — nausea, vomiting, diarrhea, constipation — that define the class.
The weight-loss evidence: SCALE
The pivotal obesity trial was SCALE Obesity and Prediabetes, which randomized 3,731 adults with overweight or obesity (without diabetes) to liraglutide 3.0 mg or placebo alongside lifestyle counseling. At 56 weeks, the liraglutide group lost a mean of about 8% of body weight (roughly 8.4 kg) versus about 2.6% (2.8 kg) on placebo.[1] Around 63% of treated participants lost at least 5% of their starting weight, and about a third lost more than 10%. This was a landmark at the time: a drug producing clinically meaningful, category-defining weight loss without surgery.
Read in 2026, though, the number is modest. In their own pivotal trials, weekly semaglutide 2.4 mg produced roughly 15% mean weight loss and tirzepatide up to about 21%. Those are separate trials with different populations and durations, so the bars are context rather than a controlled comparison — but the gap is large and consistent enough that liraglutide is no longer the front-line choice on efficacy alone.
The one true head-to-head: SUSTAIN 10
Cross-trial bars can mislead, so the most useful evidence is a direct comparison. SUSTAIN 10randomized adults with type 2 diabetes to once-weekly semaglutide 1.0 mg or once-daily liraglutide 1.2 mg. Semaglutide lowered HbA1c more and produced greater weight loss than liraglutide.[3] The comparison used a lower liraglutide dose than Saxenda, so it does not settle the 3.0 mg weight question directly — but as a like-for-like test of the two molecules, it points the same way as everything else: the weekly agent does more.
Beyond the scale: cardiovascular outcomes (LEADER)
Liraglutide's strongest standalone credential is cardiovascular. The LEADER trial enrolled 9,340 adults with type 2 diabetes at high cardiovascular risk and followed them for a median of 3.8 years. Liraglutide (up to 1.8 mg) reduced the risk of major adverse cardiovascular events — cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke — by about 13% (hazard ratio 0.87), with a significant reduction in cardiovascular death.[2]That outcome is a class-defining result, not a scale reading, and it is why liraglutide retains a place in cardiometabolic care even as the weight numbers are surpassed.
Where liraglutide still fits
The daily injection is the central trade-off. A once-weekly drug is easier to stay on, and adherence is much of what determines real-world results — so all else equal, the weekly agents win. But liraglutide is not obsolete:
- Availability and coverage. When a weekly GLP-1 is back-ordered or not covered, an available, established daily option matters more than a few percentage points on a trial chart.
- A known, gradual titration. Some clinicians and patients prefer daily dosing precisely because the dose can be adjusted in fine steps and any side effect resolves within a day of stopping rather than lingering for a week.
- Pediatric data. Liraglutide 3.0 mg is one of the few GLP-1s with a published randomized obesity trial in adolescents — SCALE Teens showed a greater reduction in BMI than placebo in 12-to-under-18-year-olds.[4]
- A long safety record. As the earliest agent, it has the longest post-marketing track record in the class, including the LEADER cardiovascular data.

The honest bottom line
Liraglutide is a genuine, trial-proven therapy: about 8% weight loss in SCALE, a real cardiovascular benefit in LEADER, and pediatric evidence few competitors can match. It is also, on efficacy, a first-generation drug that weekly semaglutide and tirzepatide now outperform — directly, in SUSTAIN 10, and by a wide margin in cross-trial weight numbers. The reasonable read is that liraglutide is no longer the default, but remains a legitimate option when a daily injection, availability, coverage, or its specific evidence base makes it the right fit.
