Our verdict
SS-31 (elamipretide) has an actual, if narrow, FDA accelerated approval for Barth syndrome — a genetic muscle disease — pending a confirmatory trial; its broader trial history in other conditions has mostly disappointed. MitoQ has no approval anywhere and rests on one positive human vascular RCT plus a mostly null recent trial record. Different targets (antioxidant scavenging vs. structural lipid stabilization), and neither has evidence for the general anti-aging use both are marketed for.
