CoQ10 vs MitoQ

Two mitochondria-targeted antioxidants with the same pitch and very different trial records — one has a positive heart-failure mortality trial, the other one positive vascular RCT and several recent nulls.

Clinical data

Q-SYMBIO showed a real reduction in cardiovascular mortality and hospitalization when added to standard heart-failure therapy — a hard outcome, not a surrogate. Migraine prevention data are real but more modest; statin-myopathy relief did not beat placebo in meta-analysis. The 'declines with age, so restore it' pitch aimed at healthy adults has no supporting outcome trial.

Clinical data

One real human RCT reversed vascular endothelial aging markers in healthy older adults, but the Parkinson's trial was a clean negative, and kidney-safety and exercise-redox RCTs since have also come back null — the vascular signal hasn't been shown to generalize beyond that one outcome.

Our verdict

CoQ10's Q-SYMBIO trial showed a real reduction in cardiovascular mortality and hospitalization in heart-failure patients — a hard outcome. MitoQ's best human result is a six-week vascular-function RCT in healthy older adults, promising but narrower, and its more recent kidney-safety and exercise-redox trials have come back null. Neither has been compared directly, and neither has outcome data supporting the general 'anti-aging energy' pitch both are sold on.
