50 Stars Health review

Flat price at every dose

A cash-pay telehealth platform selling compounded semaglutide and tirzepatide at one all-in price per plan that stays the same at every dose, plus NAD⁺, sermorelin and longevity treatments.

Our verdict

50 Stars Health keeps GLP-1 pricing simple. Each plan has one price that covers the provider consultation, follow-ups, medication, supplies and shipping, and that price doesn't change when your dose goes up. Month to month, compounded semaglutide is $146 and tirzepatide $220, with lower monthly rates if you commit to three or six months. It names its fulfillment pharmacy in its terms and lists every state it serves. The trade-offs are typical of subscription telehealth: plans renew automatically, you have to cancel at least two days before the next billing date, refunds are rare, and the clinicians are independent providers you won't choose yourself. Its semaglutide also comes mixed with vitamin B12. It suits a cash buyer in one of its 44 states who wants a predictable monthly cost through every dose increase.

What it is

50 Stars Health is the trading name of LIVV Care LLC. It describes itself as a technology platform that connects patients with independent, state-licensed providers and licensed pharmacies, and says it doesn't practice medicine or pharmacy itself. Its terms name SevenCells as its designated fulfillment pharmacy unless you choose or are allowed another. The product pages feature Dr. Gidon Akler, MD, board-certified in medical genetics and genomics. Beyond weight loss, the catalog covers longevity (NAD⁺ injections and nasal spray, glutathione, rapamycin, metformin and methylene blue), hormone treatments for men, sermorelin, sexual health, hair growth and pain relief. It's cash-pay only and doesn't bill Medicare, Medicaid or private insurance. Support is by email or phone, and you must be 18 or older to sign up.

How it works

You start with an online intake the site says takes about two minutes, covering your health history and goals, and you aren't charged unless you're approved. An independent licensed provider reviews it, usually within two days, and decides whether treatment is appropriate and at what dose; you don't pick your own dose. If you're prescribed, the order is compounded and processed in about five business days and then shipped free and discreetly, with the product pages describing two-day delivery. Your provider can add anti-nausea or anti-constipation medication if side effects call for it, and those add-ons are reviewed at a three-month follow-up. Between visits you can message the care team in the patient portal as often as you need, and your provider can adjust your dose at any time. The portal also tracks weight and daily habits.

Pricing transparency

Each GLP-1 has three subscription plans, and every dose strength is priced the same. Compounded semaglutide is $146 a month on the monthly plan, $133 a month on a three-month plan and $124 a month on a six-month plan. Compounded tirzepatide is $220, $201 and $183 a month on the same plans. The price includes the first consultation, follow-up visits, ongoing support, shipping and supplies, with no membership fee on top. Plans renew automatically, and you're charged a day before each billing cycle begins, even if you haven't completed the steps needed to keep your prescription active. To avoid the next charge, cancel in the patient portal at least two days before your billing date; cancellation takes effect at the end of the paid period. Refunds aren't usually offered, though a partial refund for a portion that hasn't been dispensed is at the company's discretion, and shipped medication can't be returned. Damaged or wrong shipments have to be reported within 48 hours.

Branded vs compounded

50 Stars Health prescribes compounded semaglutide, which it compounds with vitamin B12, in doses from 0.2 mg to 1.2 mg, and compounded tirzepatide in doses from 2 mg to 14 mg, both as injections. It says each batch is tested by its pharmacy partners for sterility, endotoxins, pH and potency. Its product pages state plainly that compounded medications are not FDA-approved and aren't reviewed by the FDA for safety, effectiveness or manufacturing. Brand-name Wegovy, Ozempic, Zepbound and Mounjaro aren't offered, and there's no oral GLP-1. NAD⁺ injections and sermorelin are sold on their own plans. The site links to LegitScript's search page but shows no certification seal. A search of the FDA warning-letter database under the company's name, legal entity and web address returned no letters to it.

Pros and watch-outs

  • Monthly plan available, with lower rates on three or six months

Watch-outs

  • Auto-renews; cancel at least two days before billing
