These are monthly price headlines; the exact order total and supply need a quote. Longevity and performance orders are not automatically refilled, while hair treatment uses quarterly refills. Its Terms also describe certain subscriptions with a three-month minimum and make product purchases non-refundable. Confirm your arrangement and age eligibility before paying.

Contour Health is a Utah telehealth practice operating as Desert Contour LLC dba Contour Health, at 1575 S River Road in St. George. It runs four programs: sermorelin under a "Performance" banner, NAD⁺ under "Longevity", compounded semaglutide and tirzepatide sold by the vial, and 3-in-1 hair-loss tablets. Its prescriptions are filled by Strive Pharmacy, a 503A compounding pharmacy it names. It accepts HSA and FSA payments with itemized receipts and a Letter of Medical Necessity on request, and displays a LegitScript seal linked to LegitScript's own checker.

Video-first and provider-led. The first consultation is a Zoom call with a licensed provider, free of charge, and the practice says you stay with the same provider for all of your care. Follow-up runs mainly through secure messaging, with replies typically within about 24 hours on weekdays, and a video visit whenever you ask. The team is named. Dr. Laarni Gonzales-Naval, a board-certified family medicine physician, is medical director; co-founder Melissa Musso, MSN FNP, runs clinical operations; co-founder Wessley Musso runs the business; and nurse practitioners Theresa Asmus, Whitney Baird and Tiffany Estera are listed with their backgrounds. NAD⁺ is self-injected under the skin, most often two to three times a week, and medication ships from the pharmacy in three to five business days. It serves 46 states and names the four it does not. The FAQ describes programs for ages 15 and older with parental and pharmacy consent, but its Terms require legal adulthood and its privacy policy specifies 18 or the state minimum. These conflicting statements do not establish peptide or NAD⁺ access for minors. The FAQ generally says pre-treatment labs are not required; ask what your own clinician requires.

Weight-loss, longevity and performance orders are charged when patient and provider decide to reorder, without auto-refill; the FAQ advises contacting the provider with about two weeks remaining.

Weight-loss cards quote semaglutide from $30 a week and tirzepatide from $50, with a two-month-vial note. The same page’s calculator gives semaglutide from $25 a week and a default tirzepatide estimate of $200 monthly alongside a $500, 20 mg vial lasting up to ten weeks. Other text says vials last six to fourteen weeks depending on dose. Obtain the actual vial quote and prescribed supply rather than assuming a fixed two-month purchase.

Hair treatment differs: tablets cost $148 quarterly and topical treatment $189 quarterly, advertised as about $49 and $63 monthly equivalents. These are quarterly purchases with an initial three-month supply minimum. The FAQ requires cancellation by email 30 days before the next charge to avoid a fee. The Terms describe certain subscriptions with a three-month minimum and 30 days’ notice; do not assume that applies to every peptide or NAD⁺ order. All product purchases are non-refundable.

Compounded; not FDA-approved. Prescription required only if a licensed provider finds it appropriate. Contour names Strive Pharmacy as the 503A pharmacy that compounds and fills its prescriptions, and states that compounded medications are not FDA-approved and not reviewed by the FDA for safety or effectiveness. Knowing who compounds yours matters, because compounded sterile products do not get the purity and potency review approved drugs do. On evidence, the two wellness programs are not one longevity offer. Sermorelin acts on the growth-hormone axis, a genuine, well-described mechanism, but long-term human outcome data stays thin and no controlled trial shows the body-composition or recovery results the category is marketed on. Injected NAD⁺ has no controlled human outcome trials at all; the one human intravenous study was a pharmacokinetic pilot showing infused NAD⁺ clears from plasma quickly. The NAD⁺ forms with human data are the oral precursors NR and NMN, and what they show is a rise in blood NAD⁺, not a demonstrated clinical outcome. The practice accepts no insurance, Medicare or Medicaid. Individual results vary.
