What it is

Inner Balance is a women-focused hormone telehealth brand founded in 2023 by Dr. Sarah Daccarett, MD, based in Boise, Idaho. Its catalogue is four prescription products: Oestra, a compounded bioidentical estradiol cream that is the flagship; Libida, a sublingual libido tablet formulated with oxytocin; a prescription sublingual NAD⁺ at 200 mg per serving; and BodyMatched, a compounded prescription face cream. Its own product page states "NAD+ 200mg per serving, sublingual for optimal absorption. From $183/mo." It accepts HSA and FSA payment, states that no insurance is needed, and offers 20% off supplements through FullScript as a membership perk.

How it works

Quiz-first and asynchronous, and the ordering matters more than usual. The brand's own FAQ describes the sequence plainly: "Start by taking our health quiz. If you're a candidate, you'll move straight to checkout. After a board-certified provider reviews your info, your custom prescription is sent to the pharmacy and shipped directly to you." That is pay-before-consult. It is not unique in this category, but it does mean the clinical review that decides whether a prescription drug is appropriate for you happens after your card has been charged, so read the refund terms as though they are the only protection you have — because they are. No baseline labs are required to start, which removes friction and also removes a data point. The founder is named and credentialed, which is a genuine advantage over the anonymous end of this board, and the operating states are not published anywhere; the only coverage claim on file is that the founder is licensed in 50 states, which is a statement about one clinician rather than about the service. The compounding pharmacy is not named, though it is described as NABP, LegitScript and PCAB certified — a description of a partner you cannot look up.

Pricing transparency

Published, and published for the right molecule: NAD⁺ from $183 a month, stated on the product page rather than gated behind the quiz. That is the case for listing it here. Three things qualify it. First, "from" is doing work — the exact plan tiers are only revealed inside the quiz funnel, so the entry figure is not necessarily your figure. Second, the guarantee has heavy fine print: refunds are limited to one per customer for life, must be requested within 14 days of cancellation and within six months of the original order, are self-pay only, and NAD⁺ is explicitly excluded from refund eligibility. Third, promotional percentages appear on product tiles without a discounted dollar figure published outside the funnel, so the advertised discount is not something you can price in advance. Before ordering, confirm the tier you are actually being placed on, and confirm in writing that you understand the NAD⁺ purchase is non-refundable.

Branded vs compounded

Compounded; not FDA-approved. Prescription required only if a licensed provider finds it appropriate. The compounding pharmacy is not named — it is described as NABP, LegitScript and PCAB certified, which is a claim about a facility you cannot verify by name, and compounded sterile and non-sterile preparations are not reviewed by the FDA for purity and potency the way approved drugs are. On the evidence, the sublingual route deserves a direct answer because it is the brand's own selling point. "Sublingual for optimal absorption" is an absorption argument, and absorption is not the thing in question. No controlled human trial shows that sublingual, injected or intranasal NAD⁺ produces a clinical outcome; the single human intravenous study was a pharmacokinetic pilot showing infused NAD⁺ clears from plasma quickly. The NAD⁺ forms that have human trial data are the oral precursors NR and NMN, and what those trials demonstrate is that they raise blood NAD⁺ — a raised biomarker, not a demonstrated benefit. Delivering a compound more efficiently does not generate outcome data for it. Separately, the flagship estradiol product is a compounded bioidentical hormone, which is also not an FDA-approved finished product. Individual results vary.
