Our verdict
Neither has a published randomized controlled trial in humans for any indication it is sold for, so neither can be called effective, and both are gray-market research-use-only products. BPC-157 has the broader preclinical resume across tendon, muscle, ligament, bone, and gut models in rats, but a 2025 narrative review concluded clinicians cannot currently recommend it for musculoskeletal injury, and the FDA flagged it in 2023 as ineligible for compounding. KPV has the narrower and cleaner mechanism, PepT1-mediated uptake into cells and suppression of NF-kB signaling from the inside, with reproducible signals in mouse colitis models. Read these as two preclinical stories with unverified supply chains, not as alternatives to a treatment.
