Our verdict
Methylene blue is a genuine drug, FDA-approved for methemoglobinemia rather than aging, with a plausible low-dose electron-carrier mechanism and one randomized single-dose brain-imaging study linking it to improved memory retrieval; its anti-aging claims are preclinical only. It also has a hormetic dose-response where high doses flip to pro-oxidant, and as a potent MAO-A inhibitor it can trigger potentially fatal serotonin syndrome with SSRIs, SNRIs or other serotonergic drugs, a documented interaction rather than a theoretical one. NAD+ precursors carry none of that: they raise blood NAD+ in randomized trials, look safe short-term, and likewise show no healthspan benefit. The two have not been compared directly.
Methylene blue fits if
Nobody should approach this as a supplement; if it is considered at all it belongs in a conversation with a clinician, and it is simply off the table for anyone on a serotonergic antidepressant.
NAD⁺ precursors fits if
Someone who wants the mitochondrial-energy idea without a drug's interaction profile, accepting that raising NAD+ is exactly where the evidence stops.
