LevelsRx review

Low-cost, oral option, phone-only cancel

A cash-pay telehealth membership selling compounded semaglutide and tirzepatide injections plus an oral semaglutide liquid, listed from $249, $349 and $269 a month and often on sale, with named pharmacies.

Our verdict

LevelsRx competes on price. Its listed starting prices, $249 a month for compounded semaglutide, $349 for tirzepatide and $269 for an oral semaglutide liquid are low for an all-inclusive compounded program, and it runs time-limited sales well below them. It says the price stays the same at every dose, names the four pharmacies in its network, and offers a needle-free oral option. The trade-offs are in the terms. The billing interval and full price list are shown only at sign-up, because the plans page its own FAQ points to doesn't exist. You're charged when you submit your intake, before a provider has seen it. Canceling takes a phone call at least seven days before the next billing date, and fees aren't refunded once a cycle has started. It suits a cash buyer who wants the lowest price and is organized enough to cancel on time. If you want an all-in price you can see before you start, or to cancel online, choose a seller that publishes both.

What it is

LevelsRx is the trading name of Trinity HealthCare Supply, LLC, which runs the technology platform and the administrative side: sign-up, billing, care coordination and pharmacy logistics. It doesn't practice medicine or employ the clinicians. Medical care comes from Usable Health, PC and its affiliated practices, whose physicians, physician assistants and nurse practitioners are licensed in your state; its site also credits OpenLoop Health with the screening criteria the intake applies, and it names three physicians on its home page. Prescriptions are filled by independent 503A compounding pharmacies its terms list as RedRock Pharmacy in St. George, Utah, Health Warehouse in Florence, Kentucky, Precision Compounding Pharmacy in Bellmore, New York, and Triad Rx in Daphne, Alabama. It says the membership is available in all 50 states. Support is by email and phone, with replies to general questions within 48 hours, and the patient portal is the fastest route for account and prescription questions.

How it works

You start with a free online assessment that takes about two minutes. A licensed provider then reviews your health history in a telehealth consultation, which may be a live video visit or an asynchronous review depending on your state's rules and the provider's judgment. If treatment is appropriate, the prescription goes to a partner pharmacy, and processing and delivery take about four to five business days. Each shipment comes with vials, syringes, alcohol pads and step-by-step instructions, or the oral liquid with a dropper. From there, the membership includes ongoing clinician support with messaging and check-ins, a personalized dosing plan, and the home page advertises a free dietitian consultation and meal options. The provider may ask for lab work, which is handled by an outside lab. If your weight falls toward the low end of the healthy range, the provider may move you to a maintenance dose or pause treatment, and if you stop, they can suggest a taper.

Pricing transparency

The home page lists starting prices of $249 a month for compounded semaglutide injections, $349 for compounded tirzepatide and $269 for the oral semaglutide liquid, and runs time-limited sales below those prices. It says the price is the same at every dose, with the provider review, plan, medication and shipping included and no separate membership fee; a comparison table on the same page gives a different starting figure, $99, so check the plan you're offered. Its terms say you're charged for the first billing cycle when you submit your intake, and refunded in full if a provider finds you medically ineligible before services begin. The subscription renews automatically at the interval shown at registration. To cancel, you must call support, at least seven days before your next billing date; email and chat requests aren't accepted. Fees are non-refundable once a cycle starts, including if you stop mid-cycle, and shipped medication can't be returned; a damaged or wrong shipment is replaced. Missed or late-canceled appointments can carry a fee. Disputes go to individual arbitration under California law.

Branded vs compounded

LevelsRx offers compounded semaglutide and compounded tirzepatide as weekly injections, and compounded semaglutide as an oral liposomal liquid taken by dropper. It states plainly that these compounded versions are not FDA-approved and haven't been evaluated by the FDA for safety, effectiveness or quality, and that they're prepared by state-licensed 503A pharmacies under USP 797 sterile standards. Brand-name Wegovy, Ozempic, Zepbound and Mounjaro aren't offered. The oral liquid is a convenience for people who dislike needles, but oral compounded semaglutide has no trial data of its own, and the brand-name oral form depends on a specific absorption enhancer, so expect results to differ from the injections. Its headline results, such as 18 percent average weight loss, come from members' self-reported data rather than clinical trials, which its own disclaimer notes. A search of the FDA warning-letter database found no letter addressed to LevelsRx.
