LIV by WellNow keeps the math simple. One monthly membership covers compounded semaglutide, supplies and unlimited virtual visits: $195 at the average 1 mg weekly dose, $265 at the higher 1.7 or 2.4 mg doses, and about $16 to $20 a month less if you pay three months at a time. It comes from the people behind WellNow Urgent Care, names the pharmacy that ships your medication, and lets you cancel online at any time. The limits are just as clear. It operates in only five states, the public pages sell compounded semaglutide and nothing else, there's no insurance billing, and subscription fees aren't refunded once charged.

Both brands are owned by licensed physicians and supported by TAG Urgent Care Support Services, LLC in East Syracuse, New York; the subscription agreement is with R Biernbaum DO, NY WellNow PC. Its chief medical officer is Robert Biernbaum, DO, an emergency physician who joined WellNow in 2021, and its director of telemedicine is Kellen Kreger, PA-C. The service is available to virtual care patients in Illinois, Indiana, Michigan, New York and Pennsylvania. Eligibility follows the standard thresholds: adults 18 and older with a BMI of 30 or more, or 27 or more with a weight-related condition such as high cholesterol or high blood pressure. People who are pregnant or trying to conceive, or who have a personal or family history of pancreatitis, medullary thyroid cancer or MEN 2, aren't eligible. The site doesn't display a LegitScript seal.
How it works
You start with an online eligibility form (date of birth, height, weight and recent GLP-1 use, then more questions). If you'd like to talk first, you can schedule one free virtual visit before signing up. A provider reviews your form, and if they have concerns they reach out to discuss; otherwise video visits aren't required. Once you're approved and enrolled, Logos Pharmacy in Tampa, Florida ships your medication by FedEx in discreet packaging, usually within five to seven business days, and refills arrive monthly. From there you have unlimited access to board-certified online providers by video, text alerts and email for questions, side effects and dose changes. There's no maximum length of treatment; how long you stay on medication depends on your goals and your provider's advice. If you have diabetes, LIV suggests talking to your primary care doctor, since brand-name semaglutide may be covered by your insurance.
Pricing transparency
The membership includes medication, supplies and unlimited visits. Monthly, it's $195 at the average dose (1 mg a week) and $265 at higher doses (1.7 or 2.4 mg a week). Paying for three months at once costs $525 or $745, which works out to $175 or about $249 a month. Tax is included, except in Illinois, where 1% is added. Higher doses need provider approval. The subscription renews automatically each month or quarter until you cancel, and you can cancel any time from your online profile or by phone, as long as it's before 8 p.m. Eastern the day before your next payment. Subscription fees are non-refundable, with no credit for unused medication, though LIV may offer a credit at its discretion. It doesn't accept insurance, but you may be able to use HSA or FSA funds. One more thing in the subscription agreement: it also lists compounded tirzepatide at $295 and $365 a month, though the public pages advertise semaglutide only.
Branded vs compounded
Compounded semaglutide, injected once a week, is what the public pages offer. LIV says it contracts only with licensed 503A compounding pharmacies or FDA-registered 503B outsourcing facilities in the U.S., and its current pharmacy partner is Logos Pharmacy. Compounded semaglutide is not FDA-approved or evaluated for safety, effectiveness or quality, which LIV states on every page. The subscription agreement also prices compounded tirzepatide tiers, so ask whether that option is open to you. There are no brand-name drugs on the menu. A search of the FDA warning-letter database under the WellNow, TAG Urgent Care and Biernbaum names returned no letters.
