MedLeanRx is a small, Texas-only service with a clear headline price: compounded semaglutide, as a weekly injection or a daily tablet, from $149 a month at the starting dose, and compounded tirzepatide from $269. It also covers the brand-name route, managing Wegovy injections or pills, Zepbound and the oral drug Foundayo for $99 a month on top of whatever the medication costs you. The supervising physician is named, reviews are promised within 12 hours, and there's no clinic visit. Two cautions. The $149 figure is for the starting dose, and the site says pricing can change with your dose, so ask what higher doses cost. And its terms are far tougher than its FAQ: the FAQ says you can cancel any time with no contract, which is true for future months, but the terms make all sales final with no refunds for consultations or subscription fees, even if you aren't prescribed. If you live in Texas and want a low entry price, it's worth a look; just treat the first payment as non-refundable.

Its terms say it connects patients with licensed providers rather than practicing medicine itself, and that Texas law governs. Besides weight loss, it runs a testosterone therapy program for men and a sexual wellness program. It's direct-pay only and doesn't bill insurance, though it says you may be able to submit receipts for reimbursement. Support is by email and through a contact form, weekdays 9 a.m. to 6 p.m. and Saturdays 10 a.m. to 2 p.m. Central time.
How it works
You complete an online health assessment the site says takes under five minutes; no appointment is needed. A licensed physician reviews your history, BMI and goals, with a target turnaround of under 12 hours. Its FAQ gives the usual eligibility guide: a BMI of 30 or more, or 27 or more with a weight-related condition, and age 18 or over. If you're approved, medication ships in temperature-controlled packaging within two to three business days, and most patients have it within five to seven business days of the assessment. After that, the physician follows up to track progress, adjust your dose and manage side effects. The promotions page works differently: you pay first, then schedule a consultation with a doctor.
Pricing transparency
The products page lists compounded semaglutide vials at $149 a month, compounded semaglutide tablets at $149 a month and compounded tirzepatide injections at $269 a month. A footnote says plans start at $149 for the starting dose and that pricing can vary by medication and dose, so the cost at maintenance doses isn't shown. For brand-name Wegovy injections, the Wegovy pill, Zepbound and Foundayo, MedLeanRx charges $99 a month for its care, and you pay for the medication separately. A promotions page offers two months of compounded tirzepatide at 2 mg a week for $325 or two months of compounded semaglutide for $300, while supplies last. The FAQ says there are no long-term contracts or cancellation fees and you can cancel any time. The terms add that all sales are final and that consultations, provider reviews, subscription fees and other fees are not refunded, and payment doesn't guarantee a prescription.
Branded vs compounded
MedLeanRx offers compounded semaglutide as a weekly injection or a daily tablet, compounded tirzepatide as a weekly injection, and brand-name Wegovy (pen or pill), Zepbound and Foundayo (orforglipron), the last three managed for a monthly fee with the drug bought separately. Its terms and promotions state that compounded medications are not FDA-approved and haven't been reviewed for quality, safety or efficacy. Its homepage blurs that line, describing the whole program as offering "FDA-approved" treatments and calling compounded semaglutide "the same active ingredient found in Wegovy", and the weight-loss estimates it shows come from trials of the brand-name drugs. The pharmacy isn't named. Its footer links to LegitScript's search page rather than a numbered certification. A search of the FDA warning-letter database under the company's name and web address returned no letters.
