SurpassMD review

Flat price at every dose

A nationwide cash-pay GLP-1 service with one price that includes the provider review, compounded medication and shipping: semaglutide $219 a month on a 30-day plan.

Our verdict

SurpassMD keeps the math simple. One monthly price covers the provider review, the compounded medication, cold-chain shipping and support, there's no separate membership fee, and the price doesn't rise as your dose does. On a 30-day plan that's $219 a month for semaglutide or $309 for tirzepatide, with lower rates for three-, six- and 12-month supplies and an introductory discount on your first subscription. It's available in every state. What you give up is visibility into who treats you and who fills the prescription: the medical group and pharmacies aren't named, intake is an online questionnaire rather than a visit, and fees aren't refunded after you cancel unless a provider disqualifies you.

What it is

SurpassMD is a brand of Life Vitality LLC, based in Lewes, Delaware. The company describes itself as a technology and administrative platform rather than a medical practice or pharmacy: care is provided by an independent professional medical corporation and its licensed clinicians through a partner telemedicine network, and medication is dispensed by licensed compounding pharmacies. Neither the medical group nor the pharmacies are named on the public site; the medical group and the providers responsible for your care are identified during intake and on request. SurpassMD serves all 50 states and Washington, D.C., though services can vary by state, clinician review and pharmacy availability. It's cash-pay only and isn't available to Medicare or Medicaid beneficiaries. Testosterone and peptide care aren't offered. Support runs weekdays from 9 a.m. to 6 p.m. Pacific. The site shows a LegitScript badge; confirm any certification in LegitScript's own directory before you buy.

How it works

Everything starts with an online assessment that takes a few minutes: your goals, health history, weight and current medications. A licensed provider from the partner medical group reviews it and follows up if more information is needed, then decides whether semaglutide, tirzepatide, oral semaglutide, another option or no medication is appropriate. A prescription is never guaranteed. Your card is charged for the first month when you finish the intake, and if a provider disqualifies you for a medical reason, you get a full refund. Once you're approved, the pharmacy ships the medication cold, discreetly and with tracking, typically within 48 hours, along with instructions. After that, the care team handles treatment questions, delivery and next steps by message. There's no scheduled video visit in the standard flow, so if you want regular face-to-face check-ins, this isn't built for that. A free nutrition guide covers protein targets, grocery ideas and meal swaps.

Pricing transparency

Pricing is laid out in a table, and the standard monthly rate depends on how long a supply you buy. Compounded semaglutide is $219 a month on a 30-day plan, $195 on a three-month supply, $180 on six months and $143 on 12 months. Compounded tirzepatide is $309, $299, $275 and $222 for the same terms. Each price includes the provider review, the medication, shipping and ongoing support, and it stays the same at every prescribed dose. Your first subscription can be lower with a SurpassMD discount code: semaglutide from $175 on 30 days down to $114 a month on 12 months, and tirzepatide from $247 down to $177. Those introductory figures are what the homepage advertises, so plan on the standard rate after the first term. A compounded oral semaglutide tablet is offered on a one-month plan at an introductory $183. It's direct pay; HSA and FSA cards may work and Klarna is available. You can cancel any time, but the request must arrive at least 72 hours before your billing date or you'll be charged one more cycle. Apart from a medical disqualification, nothing is refunded after cancellation.

Branded vs compounded

SurpassMD offers compounded semaglutide and compounded tirzepatide as once-weekly injections, plus a compounded oral semaglutide tablet for people who want to avoid injections. Compounded drugs are not FDA-approved, aren't generic versions of Wegovy or Zepbound, and may look different from the brand-name products. The company says its medications come from licensed 503A compounding pharmacies in the U.S. but doesn't name them, so ask which pharmacy will ship yours and check its license with your state board of pharmacy. Brand-name GLP-1s aren't sold here; the price table shows the manufacturers' list prices only as a comparison. Because the same price applies at every dose, tirzepatide in particular can work out well if you expect to move to higher doses. A search of the FDA warning-letter database under the SurpassMD and Life Vitality names returned no letters to the company.
