Melanotan II vs PT-141

Same melanocortin mechanism, opposite regulatory standing: bremelanotide cleared phase 3 for one narrow indication, while Melanotan II never entered that system at all.

Melanotan II

MT-2

Clinical data

Genuinely tans skin via MC1R, but it's an unapproved gray-market injectable with a documented harm record (priapism, changing moles) — and it is not the approved drug afamelanotide.

PT-141

Bremelanotide / Vyleesi

FDA-approved

FDA-approved as Vyleesi for HSDD in premenopausal women, with phase-3 evidence — but the off-label use in men and at higher doses goes beyond what's proven.

Our verdict

Bremelanotide is the active metabolite of Melanotan II, which is why they share effects such as nausea, flushing, and pigmentation changes, but the resemblance ends there. PT-141 is FDA-approved as Vyleesi for acquired, generalized HSDD in premenopausal women, with replicated placebo-controlled evidence and a label built around its transient blood-pressure rise. Melanotan II has no approval anywhere, is sold as unregulated injectable powder, and carries a documented harm record: nausea and flushing, blood-pressure and cardiovascular effects, priapism, new and darkening moles that complicate melanoma surveillance, and rare severe events including renal infarction and posterior reversible encephalopathy syndrome. It is also not afamelanotide, the approved melanocortin drug it is often confused with.

Melanotan II fits if

Melanotan II is worth understanding mainly so you can recognize its documented risks and the common false claim that it is basically an approved drug.

PT-141 fits if

PT-141 is the appropriate one to look into only inside its approved indication and under supervision, given the blood-pressure effect.
