FORM review

FORM is clear about compounding and names its pharmacy, Vios Pharmacy of Livonia, Michigan, and four of its clinicians. Its standalone compounded GLP-1s are fairly priced: semaglutide $99 for the first month and $159 after, tirzepatide $169 and then $219, with microdose tirzepatide at $199. The confusing part is how many ways it sells the same drugs. A "FORM3" blend with B12 and glycine costs more ($199 or $299 a month after a discounted first month), "Metabolic Duo" plans add a B12 vial for $299 to $399, and three-vial "stacks" run $399 to $499. The branded Wegovy pill and Zepbound programs cost a $95 monthly membership, with the medication itself coming through Novo Nordisk's and Lilly's own pharmacy programs at a price FORM does not show. Pick the exact product, get the after-first-month price, and check what the membership excludes.

FORM, marketed as FORM by GBY and based in Las Vegas, Nevada, is a telehealth store covering weight loss, peptides, longevity and hair health (hair is outside this review). Weight loss includes compounded semaglutide and tirzepatide as injections or dissolving tablets, a microdose tirzepatide program, the FORM3 triple blend, Metabolic Duo plans and multi-vial stacks, and memberships for branded Wegovy tablets (through NovoCare) and Zepbound injections (through LillyDirect). Peptides and longevity include sermorelin, a GHK-Cu cream, NAD⁺ (injection, cream or sublingual), glutathione and B12-MIC. FORM's terms describe it as a facilitator: prescribing is done by contracted medical groups and providers, and its physicians page names Armen Aivazi, MD, Alex Jabourian, DO, Amanda Little, DNP, and Bella McKenzie, ARNP. Compounded prescriptions are filled by Vios Pharmacy, 31035 Schoolcraft Road, Livonia, Michigan, named with phone and email in the privacy policy. FORM by GBY is unrelated to Form Health, a separate GLP-1 service.

You choose a pathway, complete a private online intake, and a licensed provider reviews it, typically within one to two business days. If the provider decides treatment is appropriate, the prescription ships monthly from the pharmacy; if not, you are notified and not charged. No live video visit is described. The terms list service in 48 states plus Washington, D.C., excluding Alaska and Mississippi. If a medication is unavailable through FORM's pharmacy network, you may be directed to an outside pharmacy and pay for it there. Returns are not accepted once a prescription is dispensed; damaged deliveries must be reported within three days with photos, and authorized refunds are limited to one product and can take up to 30 days.

Branded Wegovy tablets and Zepbound: a $95 monthly membership, cancel anytime; the pages say the medication comes through the NovoCare and LillyDirect pharmacy programs but do not show its cost, and the insurance FAQ says only that some plans offer partial coverage. A site-wide code takes 10% off the first month. Budget from the after-first-month figure, and note that FORM3 adds $40 to $80 a month over the standalone injection for B12 and glycine, which, as FORM's own copy says, do not make the GLP-1 work better.

Compounded semaglutide and tirzepatide (injectable, dissolving tablet, microdose and the FORM3 blend), plus FDA-approved Wegovy tablets and Zepbound injections through the manufacturers' pharmacy programs. Sermorelin, GHK-Cu, NAD⁺, glutathione and B12-MIC are compounded. FORM states on its compounded product pages that the medications are not FDA-approved and that the FDA does not review compounded products for safety, effectiveness or quality. On the evidence: the weight-loss trials behind semaglutide and tirzepatide used branded injections at full titration, and compounded tablets and microdose protocols have far less supporting data; sermorelin has a real growth-hormone-axis mechanism but thin long-term human outcome data; injectable, topical and sublingual NAD⁺ have no controlled human outcome trials; GHK-Cu has essentially no controlled human outcome data for the skin claims made.
