1st Optimal is built for people who want a GLP-1 as one part of a coaching-heavy, lab-driven program rather than a quick prescription. The $99-a-month Weight Loss Care plan covers provider visits, monthly coaching, lab reviews, supplies and shipping, and the company layers in hormone, gut-health and supplement advice. The problem is that you can't work out the total cost before you sign up. Medication is billed on top of the membership at member prices that aren't shown, the site doesn't name which GLP-1s it prescribes, the list of states is available only on request, and the membership runs for at least 120 days. Ask for the full price and drug name on the free intake call before committing. If the coaching is what you're after, the structure is genuinely more involved than a typical online prescription service, with a coach and a provider looking at your labs together.

How it works

You start by booking a free intake call or taking an online quiz; no credit card is needed to begin. After the call, you complete lab testing at home or at a local lab, and results typically take five to seven business days, longer for specialty panels. A provider then reviews your labs, history and goals in a video consultation and, if appropriate, sends a prescription to the partner pharmacy. The site says orders take about 10 to 14 business days to process and ship because prescriptions are made to order, which is slower than most online GLP-1 services. Medication is not included and its price isn't listed on the site; invoices for prescriptions, labs and supplements are processed right after your first provider visit and about three weeks before each refill. Other plans include Perimenopause Care at $129 a month, Menopause Care at $239 and a peptide plan at $109 every three months. Memberships bill monthly but have a minimum term of 120 days, and changes require a request at least seven days before the next billing date. First-time members are offered 25 percent off their first purchase by email signup. Telemedicine services aren't refundable; only unopened supplement bottles can be returned within 30 days. Payment is cash-based, with HSA and FSA cards accepted and no insurance billing.

Branded vs compounded

GLP-1 medication as once-weekly injections or daily oral tablets, sometimes combined with other metabolic agents, but the service pages don't name the specific drugs. Because prescriptions go to Belmar Pharmacy, a compounding pharmacy, they're likely to be compounded products, which are not FDA-approved; confirm the drug, form and source on your intake call. The company also prescribes hormone therapy, testosterone for diagnosed low testosterone, and peptides, and sells its own supplement protocols.
