knownwell review
Obesity medicine, billed to insurance
An obesity-medicine practice that bills your insurance, including Medicare, for live video or in-person visits and prescribes FDA-approved GLP-1s. Self-pay visits are $299, then $149.
Our verdict
knownwell works like a specialist weight clinic rather than a subscription app. You book real appointments, by video anywhere in the U.S. or in person at one of its clinics, with clinicians certified in obesity medicine, and the visits are billed to your health insurance like any other doctor's visit. The company says its average copay is $25 and there is no membership fee. Medicare is accepted too. Clinicians prescribe FDA-approved medication only, and a team handles prior authorizations so a covered drug costs you a copay; if it isn't covered, you pay the manufacturer's self-pay price at the pharmacy. The trade-offs are time and structure: appointments can take a while to get, every visit is live, and without insurance the visits alone cost $299 for the first and $149 for each follow-up, before any medication.

knownwell, inc. is a Needham, Massachusetts company that licenses its name to affiliated medical practices; the care itself comes from Seen Medical Group, P.A., Seen Medical Group of MA, P.C. and related practices. Its focus is weight-inclusive obesity medicine: the clinics advertise size-inclusive seating and private weigh-ins, and the stated approach is care without blame or shame. It offers virtual weight management and nutrition counseling in all 50 states and in-person visits at clinics in La Jolla, California; Sandy Springs, Georgia; three Chicago-area locations; three Boston-area locations; Midtown Manhattan; and Plano, Texas, where it also offers primary care. It accepts most major commercial plans and Medicare, and it treats adults 18 and older. The company says more than 25,000 patients have used it. One detail worth reading for yourself: its FAQ says it never sells patient data, including de-identified data, while its terms of use reserve the right to sell de-identified information.
How it works
You register online and the team checks your insurance before your first visit. That first appointment is a 40-minute metabolic consultation covering your health, weight history and goals; follow-ups run 15 to 20 minutes, and visits are monthly while you get started. Every visit is synchronous, either live video or in person, and you generally see the same clinician each time. No face or body photos are needed. Labs may be required before a prescription, depending on the drug and your history. Registered dietitians offer nutrition counseling, and health coaching covers sleep, stress and routines. If a GLP-1 is prescribed, the team submits the prior authorization and keeps working on it after a denial, and your prescription goes to the pharmacy that suits your coverage, which can include LillyDirect for Zepbound vials. Refills don't each need a visit, but ongoing follow-up is required. For a video visit you must be physically in a state where your clinician is licensed, usually your home state.
Pricing transparency
With in-network insurance, you pay your plan's normal copay or coinsurance for each visit, or the visit counts toward your deductible if you haven't met it. There is no membership fee. Without coverage, self-pay rates are $299 for a first weight-management visit and $149 for a follow-up, or $99 and $59 for nutrition counseling, paid when you book; knownwell doesn't offer self-pay discounts. None of those figures include medication. The company publishes the out-of-pocket drug prices patients see at the pharmacy: Wegovy pens $349 a month and the Wegovy pill $149 at the starting dose, Zepbound vials or KwikPen $299 at 2.5 mg and the Zepbound pen $499 with a coupon, Ozempic $349, Saxenda $199 and the Foundayo pill $149, with cheaper generic non-GLP-1 options as low as $10 with a GoodRx coupon. If you cancel more than 24 hours ahead, a prepaid self-pay visit is fully refunded; later cancellations and no-shows cost $50. HSA and FSA cards work for copays.
Branded vs compounded
FDA-approved weight medications only: Wegovy (pen or pill), Zepbound (vial, KwikPen or pen), Ozempic, Saxenda and the Foundayo pill, plus non-GLP-1 options such as Contrave, Qsymia and their generic components. Wegovy, Zepbound, Saxenda and Foundayo are approved for chronic weight management; Ozempic is approved for type 2 diabetes and is used off-label for weight. No compounded GLP-1 appears anywhere on its menu. Clinicians choose a drug based on your conditions, your weight-loss goal, your tolerance for side effects and what you can afford over the long run. knownwell also helps Medicare Part D members use the federal Medicare GLP-1 Bridge program. On outcomes, the homepage describes its 16% average weight loss as a 12-month figure in one place and a two-year figure in another, so treat it as a rough guide. A search of the FDA warning-letter database under the knownwell name returned no letters.

The evidence behind what knownwell prescribes
We grade the molecules, not the marketing. Here is what the human data actually shows for each thing on their menu.
- Zepbound vs Wegovy: what the trials actually show
Zepbound (tirzepatide) leads on weight loss in the one head-to-head trial; Wegovy (semaglutide) has the cardiovascular-outcomes approval. An honest comparison.
- GLP-1 cost and insurance coverage: what drives the price, and what's actually covered
List price, net price, and your price are three different numbers. Indication drives coverage, savings cards have fine print, and cost is itself a clinical variable.
- Orforglipron: the oral non-peptide GLP-1, and what the trials actually show
A once-daily small-molecule GLP-1 pill with no food restrictions — from Phase 2 proof of concept to the Phase 3 read-outs. The evidence, graded.
