Primary Clinic review
Primary care, GLP-1 at cost
A virtual direct primary care membership at $89 a month that lets members buy compounded semaglutide and tirzepatide at wholesale prices.
Our verdict
Primary Clinic isn't a weight-loss app; it's a virtual primary care practice that also prescribes GLP-1s, and its weight program, Direct GLP, only makes sense if you value the primary care. For $89 a month you get unlimited virtual visits, texting with your provider, routine labs and a list of common medications, and then you buy compounded semaglutide or tirzepatide at what the practice calls wholesale cost, from $85 a month for low-dose semaglutide. At low doses the combined bill of about $174 a month is competitive, and at higher doses it stays reasonable. The catches are the three-month minimum on the membership, an early-termination fee if you leave sooner, and a footprint of 17 states and D.C. If you already have a doctor you like and only want a GLP-1, an all-in program may be simpler.

Primary Clinic is the trading name of Georgia Primary Care LLC, a direct primary care practice whose terms list an office at 1611 South Utica Avenue in Tulsa, Oklahoma. Direct primary care means you pay a flat monthly membership instead of using insurance for routine care. Primary Clinic delivers that care entirely online, through the Charm patient portal, and treats patients aged 12 and older. Members get unlimited virtual visits, direct text access, routine labs including an annual comprehensive blood panel, and unlimited formulary prescriptions filled through ManifestRx, with up to six free mail-order shipments a year. The membership doesn't replace insurance for emergencies, specialists, imaging or hospital care. Direct GLP is the practice's weight-management offer within that membership. The sign-up form lists Arizona, Colorado, D.C., Florida, Georgia, Iowa, Idaho, Kansas, Montana, Nebraska, New Mexico, Nevada, New York, Oklahoma, Oregon, Utah, Virginia and Washington.
How it works
You start with a free virtual consultation by phone or text, then join the membership. A provider reviews your health history and BMI; generally, GLP-1s are prescribed for a BMI of 30 or more, or 27 or more with a weight-related condition. Lab work is drawn at a lab near your home and is covered by the membership. If medication is appropriate, it ships to your door from a licensed 503A compounding pharmacy. From there, the same provider manages your dose, side effects and labs as part of ongoing primary care, and you can reach them by text, video or phone; the practice's main site says first visits run 30 to 60 minutes and prescriptions are usually sent the same day. If you don't qualify for a GLP-1, the primary care services remain available to you.
Pricing transparency
The membership is $89 a month, and the practice's main site advertises the first month free. GLP-1 medication is paid separately and is available only to active members. Compounded semaglutide is $85 a month at 0.25 to 0.75 mg, $108 at 1 to 1.25 mg, $145 at 1.5 to 2 mg and $183 at 2.25 to 2.7 mg. Compounded tirzepatide is $133 a month at 1.25 to 2.5 mg, $183 at 4 to 5.5 mg, $233 at 6 to 7.5 mg and $258 at 8 to 10 mg. So the combined cost starts at about $174 a month and reaches about $347 at the top tirzepatide tier. The membership terms require a three-month minimum, with an early-termination fee equal to the rest of that term, after which it renews month to month until either side gives 30 days' written notice; the main site's "cancel anytime" line doesn't reflect that minimum. No insurance is accepted, and disputes go to binding arbitration.
Branded vs compounded
The practice prescribes compounded semaglutide and tirzepatide made by licensed 503A pharmacies for individual patients. It says they contain the same active ingredients as the brand-name drugs but are not FDA-approved, and its consent form warns that changes in regulation could interrupt their availability. It doesn't name the compounding pharmacy for GLP-1s, and brand-name Wegovy or Zepbound isn't listed as part of the weight program. Because GLP-1 care sits inside ongoing primary care, the same provider can watch your blood pressure, blood sugar and other conditions as you lose weight, which is the model's main clinical advantage. Its site doesn't display a LegitScript seal. A search of the FDA warning-letter database under the Primary Clinic, Direct GLP and Georgia Primary Care names returned no letters.

The evidence behind what Primary Clinic prescribes
We grade the molecules, not the marketing. Here is what the human data actually shows for each thing on their menu.
- What the semaglutide weight-loss trials actually showed
STEP 1, STEP 4, and SELECT — the efficacy, the rebound, and the cardiovascular signal, read straight from the primary literature.
- Tirzepatide vs semaglutide for weight loss: what the trials actually support
Most comparisons are cross-trial. Only SURPASS-2 and SURMOUNT-5 randomized one drug against the other — here's the precise distinction.
- Compounded vs branded GLP-1: regulatory status, sourcing and the evidence
Compounded GLP-1 isn't FDA-approved, the shortage exception behind it ended in 2025, the FDA moved against mass-marketed copies in 2026, and the trial data belong to the branded product. Here's the precise picture.
