Alternatives to Wegovy

Alternatives to Wegovy, and what switching to tirzepatide costs

Wegovy is already approved for weight management, so the reason to look elsewhere is usually cost or results, not the label. The table answers the cost side directly: for each service that prices both molecules, it shows what tirzepatide costs next to semaglutide.

Short answer

Zepbound (tirzepatide) is the approved alternative with the larger average weight loss in a head-to-head trial. Across 47 services that publish month-to-month prices for both compounded molecules, tirzepatide costs a median $70 a month more than semaglutide; 2 charge the same or less. Bought from the manufacturers, the Wegovy pen is $349 a month and Zepbound vials $299 to $449.

The shortlist

47 providers, side by side

Included: services that publish a month-to-month price for BOTH compounded semaglutide and compounded tirzepatide, so the switch can be priced at one service. The Access cell gives both figures and the difference. Partners are listed first; within each group, the smallest tirzepatide premium comes first.

We never show a fabricated price or rating. Facts are shown only where a provider publishes them.

Wegovy or Zepbound: what the trial and the price lists say

SURMOUNT-5 is the one trial that randomized people with obesity to Zepbound or Wegovy. At 72 weeks, average weight loss was 20.2% on tirzepatide and 13.7% on semaglutide. Earlier trials of each drug against placebo (STEP 1 for Wegovy, SURMOUNT-1 for Zepbound) pointed the same way. Our Zepbound vs Wegovy evidence review covers side effects and who stopped treatment.

On price, the two manufacturers sell to self-pay patients in different ways. Novo Nordisk’s self-pay Wegovy pen is $349 a month at standard doses (after two months at a lower rate) and $399 for the 7.2 mg pen. Lilly’s Zepbound vials are priced by dose: $299 at 2.5 mg, rising to $449 at 7.5 mg and above, per 4 weeks. So the approved alternative can cost less than Wegovy at the start of treatment and more at the top of the ladder. Sources and dates are in the GLP-1 cost estimator.

The compounded routes, and what the price gap tells you

Telehealth services also sell compounded semaglutide (Wegovy’s molecule) and compounded tirzepatide (Zepbound’s). Neither is FDA-approved, neither is equivalent to the brand, and the trial figures above were measured on the approved drugs. Their appeal is a lower cash price at higher doses, often flat at every dose.

The gap column is the useful number if you are deciding whether to switch molecules rather than services. A small or zero gap means the same service will move you to tirzepatide for little or no extra cost; a gap of $100 or more means the switch is a budget decision as well as a clinical one. The prices themselves, sorted, are on the cheapest tirzepatide and cheapest semaglutide lists. A third option for some readers is staying on Wegovy through insurance: plans that cover anti-obesity drugs, with the manufacturer’s savings offer, can bring it down to a copay.

How we judge providers

  • Pricing transparency — is the all-in monthly cost clear, or does a low headline hide consults, labs, and medication fees?
  • Clinical oversight — real clinician review and dose titration, not a rubber-stamp questionnaire.
  • Branded vs compounded clarity — does the provider tell you plainly which you’re getting? (It matters — see our compounded-vs-branded explainer.)
  • Honest insurance handling — straight answers about what is and isn’t covered.

We never show a fabricated price or rating, and access notes appear only where a provider publishes them. Pricing and availability change often — confirm current details on the provider’s own site.

Keep comparing

Weighing the brands themselves? Read how to get Zepbound and the Wegovy cost breakdown. Want a pill instead of an injection? See Ozempic alternatives, which covers the oral routes. Every service with tirzepatide, priced or not, is on the compounded tirzepatide providers page, and the full roster is in the GLP-1 provider roundup.

Choosing between two services? Every provider head-to-head is on one page, including Found vs PlushCare, Hims vs PlushCare and Strut Health vs FORM Health.

Common questions

Is Zepbound better than Wegovy for weight loss?
In SURMOUNT-5, the head-to-head trial, people on Zepbound lost 20.2% of body weight on average at 72 weeks against 13.7% on Wegovy. Tolerability, cost and coverage decide which is right for a given person.
Is Zepbound cheaper than Wegovy without insurance?
At the 2.5 mg starting dose, Lilly's self-pay vial costs less than the standard Wegovy self-pay pen; at 7.5 mg and above it costs more. The short answer at the top of the page has the current figures.
How much more is compounded tirzepatide than compounded semaglutide?
It varies by service. The table shows the difference for every service that publishes both month-to-month prices, from no difference to well over $100 a month.
Can I go from Wegovy to tirzepatide directly?
A prescriber can switch you, but the doses do not convert: Wegovy runs from 0.25 mg to 2.4 mg and tirzepatide from 2.5 mg to 15 mg, so the new drug gets its own starting dose and titration.
